Feel free to fill out the form below if you'd like to use the truck! Please provide the following information for the Driver of the truck. We will need a copy of a valid drivers license and auto insurance card, your insurance is Primary. Driver's Name: Current or New Address: City: State: Zip: Phone Number: Please read and understand the following terms and conditions for the use of "The Woodlands Team Truck". I understand that any and all drivers must be at least 25 years of age. "The Truck" is limited to The Woodlands area. Return "The Truck" in the same condition you found it, FULL GAS TANK, Clean and all equipment in the truck (hand truck & furniture pads). Please report any mishaps to our office and to the appropriate law enforcment authorities immediately. Contact us: (281) 362-2567 and/or (281) 362-2502. Please report any mechanical issues you think we may need to have checked. Such as low oil level, sounds from engine and ect... Be sure to check for personal belongings you may have left in vehicle upon exiting. We are not responsible for items left, damaged or stolen during the use of "The Truck". Familiarize yourself with all the controls before moving the vehicle. Read all warning decals posted on vehicle. Comply with all state and federal regulations, including the use of seatbelts. No more than two occupants in this vehicle. Be especially aware of the width and height of this vehicle. You can cause severe damage by underestimating clearance. Please do not drive through restaurants, banks, overhangs, and canopies. CHECK YOUR CLEARANCE! 10' 6". Use the tied down slats to secure your load. Severe damage could occur to the vehicle or your posessions if not tied down properly. Do not overload vehicle as steering and braking characteristics change. Always use the emergency brake properly. Always lock the roll up door, and make sure ramp is latched in place. Respect this vehicle. Drive cautiously and courteously. Remember, you are responsible for all incidents. This vehicle requires additional distance in slowing and stopping than the vehicle you are used to driving. A loaded vehicle takes longer to stop.
